Completing the questionnaire. The best way to answer this questionnaire is to use our interactive version.
Download the questionnaire using the link below. Save the questionnaire file to your computer, then open it using Adobe Reader software. You may be prompted by your computer operating system to upgrade to a more recent version of Adobe Reader (this is free software). The file may also be compatible with your version of Adobe Reader. Contact the project team if you cannot open the file.
The questionnaire is not compatible with other applications that can open pdf files. For example, the Preview application on Mac computers will not render the questionnaire correctly. You can download the most recent Adobe Reader software for almost all operating systems here.
Note: Once opened, the file will accept information. It has been designed to make it easy for you to complete the questionnaire electronically.

If you prefer not to use the interactive version, you must contact the project team and request a hard copy of the questionnaire. This is the only way to obtain a hard copy of the questionnaire; if you print out the interactive version you will not have access to drop-down menus and critical information will be missing.

No. 332-543) was requested by the House Ways and Means and Senate Finance Committees (Committees) on August 2, 2013. The Committees have requested that the Commission transmit its report by December 15, 2014. In their letter, the Committees asked the Commission conduct a survey of U.S. firms to better understand the impact of policies in India that discriminate against U.S. exports and investment.
The Commission has designated as "confidential business information" the information you provide in response to this questionnaire, to the extent that such information would reveal the operations of your organization and is not otherwise available to the public. The Commission will not disclose such confidential business information except as provided for in section 7 of the questionnaire. Information received in response to this questionnaire will be aggregated with information from other questionnaire responses and will not be published in a manner that would reveal the operations of your organization.
